Liability & Hold Harmless Forms
We’re so glad you’ll be joining us for Kidzz Helping Kidzz activities! To help us ensure a safe and positive experience for everyone, we ask that you complete the following forms before participating:

Liability and Hold Harmless Agreement: This form is for adults who will be participating. It releases Kidzz Helping Kidzz from any liability in the event of personal injury or property damage.

Minor Participant Form: This form is for children taking part in our activities. It releases Kidzz Helping Kidzz from any liability related to the child’s personal injury or property damage.
Each form includes areas to initial, as well as a signature and date section at the bottom. Once completed, you can either email the forms to info@kidzzhelpingkidzz.org or bring them with you in person. After clicking on a button below, a new tab will open in your web browser where you can download the PDF, sign it from your device and then email back to us.

We are Kidzz Helping Kidzz!
Kidzz Helping Kidzz is a youth-led nonprofit organization dedicated to brightening the lives of children experiencing challenging times, such as hospitalization or food insecurity. Founded on the principle of kids helping other kids, the organization is driven by young volunteers who are passionate about making a difference in their communities.
Through initiatives like organizing toy drives to bring smiles to children in hospitals and partnering with programs to provide essential food resources, Kidzz Helping Kidzz delivers moments of hope and joy. The organization not only provides tangible support but also inspires a culture of kindness, empathy, and mutual support among young people, empowering them to become active agents of positive change.
